Cannot create new record within Form

I am having an issue where we cannot add new records within a form. The form is located both locally and over the network via network shares.
Whenever we open the Form, it'll open Base just fine, and we can see the Form in the Forms field, but after opening the Form from there and trying to create a new record it gives the error message in this photo:
What could be causing this issue and how would I go about resolving it?
thanks

The error message tells it all. It does even tell you what you are supposed to do.

Well golly-gee-willickers! Why didn't I think of that.
1) you cannot just simply enter data into the specified field
2) it happens when it shouldn't and it's random, meaning it can happen and then another occasion, not
I'm also getting an error on another unit that says a specific user "cannot be found."
Any ideas other than smart remarks of pointing out the obvious will be greatly appreciated.
Thanks

Since you're running in a client-server environment, perhaps using a
standalone Form, this could be caused by a variety of things including OS-level: file locking, database-level: user-roles, operating mode (single- or multi-user), table-level: constraints, referential integrity, lack of an auto-incrementing primary key...etc.
For starters, can we presume you're running a the database in a proper multi-user mode through a
database-server like HSQLDB, MySQL, etc?
Otherwise, if you're accessing a single-user, 'embedded database' stored within a Base .odb, then you'll be fighting OS-level file locks if you attempt to open the Form on multiple computers simultaneously, or even if you're running the Quickstarter on any connected computer used for database access.

And why don't you tell us? The most trivial reason would be that you simply forgot to add the form control for that mandatory field. It could be anything but you do not even tell us the database you are working with.

Sorry, I'll clarify a little more. The "form" that I was referring to that is located both locally and on the network was the .odb file but that only opens the application and connects to the HSQLDB.
And yes, it is being run as a HSQLDB. I meant to include that originally. Also, worth noting, it isn't access permissions (I don't think) because it happens even when attempting to access it locally.
