[Dropped] Avery label prints incorrectly; Print Preview OK

Im trying to use the boxes of Avery Pres-A-Ply Pin-Fed Computer Labels (size 15/16" x 3-1/2") #30720 I have before switching to something better suited for my Brother MFC-J497DW Printer.

These worked perfect when using MS office before and after switching to OO The Avery 4013 template I could find isnt working correctly.

The first three labels (of 12) are blank and the print whole is shifted 1/2 a label down.

I tried to (crudely) correct this in the V2 version attached. Still the first 3 are blank and the bottom one is clipped in half. The bottom looks like its hanging under the bottom field of view so to speak.

I can get a picture of the misprint if that would help

Can anyone help with this or point me int he right direction?

Where did you find this label template?
I do not see it in the list.
Please attach a screenshot of the wizard.

That displacement suggests that the paper size in the printer may not be set to the same size as the Page size in the Template you use - one as US Letter, the other as A4. In OpenOffice check /File /Print : Properties button. This is necessary to "tell" the printer what paper size to use.
