[Solved] Format reports from Calc

I need to create a formatted report from my OOCalc rows. The report needs to be in OOWriter or some other text app. Here's my spreadsheet (it's all straight text with column titles in row 1):

Name          Address          City          State  Phone
John Smith    100 Main St      Springfield   IL     328-874-0868
Jane Brown    13 Wash Blvd     Bluff         OK     601-591-3422

Here's what the report needs to look like:

John Smith
100 Main St
Springfield IL
328-874-0868

Jane Brown
13 Wash Blvd
Bluff OK
601-591-3422

Does anyone have a straight forward way to do this?

Writer Guide, Chapter 11: Using Mail Merge
Mail Merge reads a data source (your spreadsheet) and creates a Writer document from a Writer template formatted the way you specify. Please read the documentation above before asking further questions since it will probably answer most of them. One very easy way to produce your report is to treat this as a label creation task. Then you can send the report, instead of to sheets of labels, to a Writer document or PDF or to sheets of paper in your printer.
