[Solved] Insert Header and Footer in Database Form

Hi. Anyone know how to insert header and footer in a form? Need help please.

Open the form in design mode. It's an ordinary Writer document in "Web Layout View".
Switch from View>Web Layout to View>Print Layout and save the form in this view.
