Confusion on database fields

I have a database that includes physician and physician number. I made a report and a query also that is supposed to pull up the physician's name, but instead it is pulling up the physician's phone number. Can you help me figure out how to get it to pull up the name instead?

Change your query to contain required data and create new report.
Or add your file here and we could help you to seeing into your file.

The query contains the required data and it still pulls up physician number. I have deleted it and tried it again and it didn't work either.

Post a small section of file, with suitable anonymous data.

I don't know how to do that without breaking hippa policy...

Save a copy of the database. Delete all the records. Add fake records. Post that one.

Another issue I can not understand is why everybody describes database queries and spreadsheet formulas instead of posting them.

I don't know how to post formulas or even where to find them....I am learning how to do open office from scratch and knew nothing about it to start with.
