[Solved]No Crash but forms in Nirwana

Hi,
just spend a couple of hours creating a report menu form and a generic form template. All saved and working fine until I restart the db and notice the forms have vanished.
Well, the template has vanished completely - no more sitting in the db container whereas the afrmReports is there but opens up with the error message as shown below.
Opening the form in design view will open some sort of blank which I think may be Writer ?
Anyone experienced something similar?

A clean reinstall with killing %appdata% LibreOffice folder as well fixed it.
