Form for inserting data to table from fields

I would like to create form for inerting data to table but from fields in form. Additionally it should have categories list which shows categories names from Category table but it would insert to Products category ID, not name.
I would like to read data from field in form and add these values into table.

Is this possible in Libreoffice/Openoffice base?

See Ch8 - Getting Started with Base has a chapter on creating forms. Also, look in the Tutorials and Examples areas of this Forum.
I included a simple demonstration database with 2 forms that both use a ListBox to display/select a category (Studio)..
