Could someone please help me reproduce a form that's smudged

Hi
I have a form I just need it reproduced as it is as the original got wet and doesn't look so great
I can email it to anyone willing to help

Are you looking for something to print and fill out on paper, or something that will be completed electronically?

Ok, so I'll move the topic to the General Discussion area, since this doesn't require a database form.

If it's just one page, I'd be willing to take a crack at it. Click the "email" link at the right to send me a message and I'll reply with an address you can send an attachment to.

Would it not be easier to telephone Cigna and ask for another copy of the form?
