I have set autocorrect to No replacement and " (standard quotes) and ' (standard apostrophe) as default.
But apparently sometimes it simply reverts to smart quotes. And I have to go back and do a search and replace
Why does this happen?

I think it is not the best idea to ignore the language setting for the all text in a document.
You can modify tha settings for the typed-in apostrophes from the menu:
Tools - Autocorrect Options - Localised Options (TAB) - Simple quotes/Double quotes area.
