Make entered text show up in multiple places in a single doc

I would like to know if someone can help me with this. I am creating a single page with numerous text boxes that are used for my workplace. I would like to create two forms on the page. Some information in one form is also included in the second form. Patient Name, Prescription, equiptment, etc. I would like to have the ability to enter the information on only one form, and have the other auto fill the same fields. In short, I want to enter text in one place, and have it show up in more than one place. How can I accomplish this with Openoffice?

You can use the 'Document properties - Custom properties' for this task. It is a 'mini database' inside the document.
You can insert these information into the document as text fields.

It works fine with the native, international Standard .odf file format.

I think I need to explain a little better. The document form will be saved in a "Blank" state with no information in the text boxes. The information to be entered will be different for each patient. I am including an attachment of the first form to be filled out. I would like some of this information to "autofill" on another form that will be added on the right side of the page. The intention is that after printing, the page can be cut in half and used differing functions for the same patient.

Another way:
You can achieve this task with the Calc application. (You can format that document similar to this Writer document, and you can reference to an another cell.)

I can not see any FIELDS in your example documents. You need study and apply this function (Insert - Fields...)

The basic idea is that you enter the data once into the database (or spreadsheet). You then display what you have enterrred in multiple places in the document (or spreadsheet).

Can I suggest you check the up to date Writer guide and/or the OOo v3.3 Writer Manual for more assistance.

I followed Tibor's advice and remade the form with the calc spreadsheet. It is working well now. Thank you all for your advice and assistance.
