I want to design a E-Form. A form which includes header and footer on the page and a Nice presentable forms.
I also want to insert background transparent image.
Can somebody provide me the tips for that.

How will be filled in? Within OOo? Or exported as PDF form? Or printed out and filled in manually?
How will collect the data from form? In database, by emails?
