Install https://libreoffice.org/download/ instead of OpenOffice.

See [Tutorial] Differences between Writer and MS Word files for why you should always work in and save files as .odt and a discussion of possible difficulties.
LO 6.3.5.2, Windows 10 Home 64 bit

See the Writer Guide, the Writer FAQ, the Writer Tutorials and Writer for students.

Remember: Always save your Writer files as .odt files. - see here for the many reasons why.

What you are trying to edit is a set of form controls attached to a Word document. OpenOffice has never implemented the abilitiy of accessing these attached objects. LibreOffice is the actively developed successor to OpenOffice. It can handle attached form controls in MS Word documents and the latest version 6.3 improves this capability. OpenOffice is almost dead.

Of course it is a severe mistake to send such input form attached to Word documents, assuming that "everybody" must have that program which is not even availlable for Linux platforms. Even Word users should be able to convert their Word forms to PDF forms which could be filled out on any device having a decent PDF tool.
