Headers/footers lost when exporting to PDF

When I export a chapter from .odt in Apache Open Office (on Windows 10), I get a nice looking .pdf file, but lose headers/footers. No indication of whether the page is left or right. My POD company insists on a .pdf file. I can't do pagination or index until all 19 chapters have been transformed into a single pdf. Can't begin marketing until ready to print!

Just a tip:
Do you use the Export PDF function? Or you use a virtual PDF-printer software?

The Export PDF/Selection works fine for me (as you expected: the PDF will be exported with the footer and header texts) in my LO 6.1.6 x64.

I used the Export as pdf, in Open Office.

Try this .odt and the PDF created from it.
