Documentchanges saved in new file

Help me please. When I change the text, when saving, the program makes the changes save not in the document, but in a new file. this is not convenient for me. Maybe I'm doing something wrong?

Does the Save process ask you for a new filename? If so, it may be that you are using Save As instead of Save.

What are the exact steps you use to save a file where this happens?
