You should start by deleting or renaming your OO user profile. As you are using Windows close OpenOffice and the Quickstarter. Open File
Explorer. Copy and paste %appdata%\OpenOffice\4 in the File Explorer (sometimes called Windows Explorer)
Address Bar and press Enter. Rename the "user" folder - "user.old" is a good choice. Start OpenOffice.

When spellchecking is working right clicking on a word will offer a Synonyms option (towards bottom of popup). Clicking on this will offer the actual alternate words and a thesaurus link.
Apache OpenOffice 4.1.3 on Xubuntu 16.04 (mostly 64 bit version) and infrequently on Win2K/XP

RoryOF gave you the steps; I'll repeat them as a list if that helps.

    Close OpenOffice and the Quickstarter.
Open File Explorer (sometimes called Windows Explorer)
Copy and paste the text %appdata%\OpenOffice\4 in the File Explorer Address Bar - not the search bar - and press Enter.
Rename the "user" folder - "user.old" is a good choice.
Start OpenOffice; you should be prompted to enter your name and initials; if not, something in the steps above was missed.

If this doesn't resolve spell check problems, see the following topics:

[Troubleshooting] Spell check in OpenOffice.
