Firstly, I'll apologize if this is redundant; I didn't see a related thread.
So, I have created a checklist form with checkboxes at each item. I have also exported this as a editable PDF. I am using this form on an iPad.
During use, checkboxes remain checked, but when the form is saved, either overwriting the original or as a new document, the checkboxes return to an unchecked state.
Form attached.
All I'm looking for here is the retention of checked or unchecked boxes once the form is saved.

Your sample file works for me on my Win10 x64 Prof., with the free Adobe Reader software.
It may depend on the capability of the PDF reader (editor) software.
