Access 2013 Table Relationship Issue

I have an Access 2013 database that joins 5 tables. The problem I am having is that a form called "Audit Form" does not allow me to enter data into some of the fields on the form I am pretty sure it is a relationship issue. I am very new to Access and do not know VBA. I would be grateful for any help in looking at the database for issues. It is a rather small database. If you provide an email address or explain how to attach the database I would send it ASAP.

¿ Have you defined your Access Database in either ? :

1. OpenOffice Base
2. LibreOffice Base

If not, it might be best if you look for another forum that can help you with your Microsoft Access database.
