Date field malfunction

I'm using a mysql form which worked fin until recently. The date field displays properly however when I export the date changes to "11/30/13". This seems to happen when I autofocus to the next field and the date autocompletes. When I insert the date manually and save the record again the date then exports properly. The date field in mysql records properly. Could be some conflict between the form date field and field type in mysql. Not sure.
