How to use the Form Property?

Many people are having great difficulties using the Form Property in order to acquire or submit data to and from Calc.

Even in thie Base forum, I found so many related questions.

Answers are rarely found in Help menu or in this board.

Please help us find the simple and quick solution by using the following example:

Desired outcome: Click one of the buttons and show the selected data in the blue area (A11:C11)

Question: I don't know what to enter in URL field

In this DATA tab, there are so many fields for which there are no tutorials or specific help menu.

Question: How can I acquire the button-selected data using this Form Property window?


Essential Issue: I would like to show the button-selected data in the specified field in Calc window
WITHOUT using VBA.

Reason: For security reason, macro is prohibited in my environment.

I didn't know how to handle checkbox because the result returned by the checkbox was non-numeric
and could not cite other location.
