Create an ID template with a replaceable image

I want to create a template used to store ID information. I have it setup with all the fields/text I want, and I have an image placeholder (a silhouette which will be replaced by someone's actual picture). I can't figure out how to make the image replaceable. Ideally I would like to be able to click on the image and open an dialog to select a different image. Is this possible? Alternatively, just being able to modify which image is put there would be nice. Any ideas?

1) A directory of picture files.
2) A database table of persons with a field of picture file names.
3) A Writer document with a form where you pick a person from a table and display the related picture file in a picture form control.

Yes, number 3 is what I want to do. Sort of. picture can just come from a directory. How do you get an image on the form that you can replace by clicking on it. Thanks.

Point 3) requires 1) and 2). The database is the crucial point where you map the picture files to additional information (text, numbers, dates) and access data through forms. Then you can attach a form to an arbitrary office document. The form in my database document is a Writer document. It could be a stand-alone Writer/Calc document as well.

So you can't swap a picture without a database?
