Get "new from template" button back in LO 4.2

Just installed LO 4.2.x from the ppa onto kubuntu precise.

Lost my New From Template button.

How do I get it back? I didn't see it when I looked in the usual places.

I use it a lot.

Do not have the "Templates" button? It's the second from the top in the main window.

Sort of (see below), that's still in the list as before, but the "New From Template" button did it all in one mouse click.

Actually, I just checked the "Template Manager" button in Visible Buttons. It appeared on my Standard toolbar for a few milliseconds and then went invisible. Going back to the Visible Buttons, it has unchecked itself. I don't see a "Templates" button anywhere.

Edit:
Just found "Templates" at the bottom of the New Document drop down list. It's only 1 extra click, but I'd still like to put it on my Standard toolbar. Template Manager button is still misbehaving.

Hi. I now have LO 4.2.5 and can find no way of adding the command to a menu or tool bar. The command is NOT shown in any of the categories in the left panel of Tools > Customise > Add. I wish I could install AOO.

Hello. I am creating a new profile folder and discovered that you can add an icon to the tool bar in the start window, that is, the window which remains when all documents are closed.

Why not simply hit Shift+Ctrl+N?

The OP was after a tool bar icon that used to be in the software.
