[Example] Adding Option Buttons to a FORM

Since no one was able to offer an example.odb per my original request [15-dec-2014], I used the suggested 2010 tutorial from "Sheepdog" and set up an example.odb [see attached]. The hope is that it will be useful to other people.
Note that the process I used to add Option Buttons is included in the attached .odb
