Reinstall?
(Uninstall - delete the user profile - reinstall)

Most of the extensions are third party softwares. Try to ask the author.
That latest version of the extension seem 10 years old. Maybe it is not compatible with the newer versions of the AOO/LO.

That extension is written in basic so if Zizi64's suggestions don't work - the source is there for you to try a fix yourself.

If it stopped working by a mere minor update, then something has been broken in the process. You should ask the dev mailing list if something has been indeed changed, intentionally or not in that regard.

I remember it happened once with LO but they had warned that macros and extensions would have to be updated (I had to adapt some of my macros indeed).

The ASCII table subs aren't simple, involving a grid control and quite a bit of code creating it.

You have the option to revert to a previous OO version. There seem to have been several threads complaining about the latest update - which has persuaded me to hold off on it.
